Submit on-line request for return authorization
This RA form is to be used only by customers in the USA returning instruments to the Endress+Hauser factory for repair, calibrations, upgrades or check-out (troubleshooting).
Be advised that there is a minimum $180.00 check-out fee for instruments we inspect and find no problems, or is a non-repairable item.
The information you will need to complete this form:
Required:
- Customer name
- Customer shipping address
- Customer billing address
- Customer phone number and/or e-mail address
- Customer contact
- Equipment type
- Reason for return
- Failure description if applicable
- Process material(s) to which the equipment has been exposed.
- Materials used to decontaminate and clean equipment
Desirable:
- Purchase order number (to help us get your instrument back to you quicker, please provide us with PO number up front) This will also allow you to track your shipment using our on-line order tracking tool.
Repair Requests
The on-line process is the quickest and most efficient way to obtain an Return Authorization (RA) number. Over the next several screens you will fill in all of the required information, and at the end, your RA number will be displayed, and if you provide an e-mail address on the form, you will also receive a confirmation message with your RA number. Calibration Requests
Please call the repair/calibration coordination team at 800-642-8737 and select option 1 to create return authorization for instruments requiring calibration. Click on "Calibration Data Sheet" link and complete a form for each type of instrument to be returned for calibration. Ask your repair and calibration coordinator for specifics regarding documentation requirements.
NOTE: Before Proceeding:
Be sure your instrument is one which Endress+Hauser can repair or calibrate at the factory. Some items are non-repairable, or are only serviced in the field by electronics module replacement.
